Back Or Neck Pain Is Common – But Some Symptoms Need Urgent or Prompt Assessment
Most episodes of back and neck pain are not caused by serious problems and improve with time and appropriate management.
However, there are some symptoms that should never be ignored because they may indicate a condition requiring urgent medical assessment.
Fortunately, these conditions are uncommon, but recognising them early can be important.
1. Loss Of Bladder Or Bowel Control
If you suddenly develop:
- Difficulty starting to pass urine
- Loss of bladder control
- Loss of bowel control
- Reduced awareness of bladder filling
alongside back pain or leg symptoms, urgent medical assessment is required.
These symptoms may be associated with a rare condition called cauda equina syndrome, where nerves at the bottom of the spine become compressed.
2. Numbness Around The Saddle Area
The “saddle area” includes:
- Genitals
- Buttocks
- Inner thighs
Loss of sensation in this area should always be assessed urgently, particularly when combined with back pain and leg symptoms.
3. Progressive Weakness
Weakness that is worsening over time should never be ignored.
Examples include:
- Foot drop- Difficulty lifting the front of the foot
- Difficulty performing a heel raise/ lifting the heel off the floor
- Increasing leg weakness
- Loss of grip strength
- Increasing difficulty climbing stairs
Progressive neurological changes warrant medical assessment.
4. Problems With Balance, Walking Or Hand Dexterity
People often assume these symptoms are simply part of ageing.
However, symptoms such as:
- Frequent stumbling
- Loss of balance
- Dropping objects
- Difficulty fastening buttons or jewellery
- Changes in handwriting
can sometimes indicate Degenerative Cervical Myelopathy (DCM), a condition affecting the spinal cord in the neck.
Because DCM often develops gradually, diagnosis can be delayed.
5. Severe Pain Following Trauma
Back or neck pain following:
- A fall
- Road traffic collision
- Significant impact injury
should be assessed appropriately, particularly in older adults or people with osteoporosis.
What Should You Do?
If you experience any of the symptoms described in this article, the key is to match the urgency of your symptoms to the appropriate level of care.

- New loss of bladder or bowel control
- Difficulty initiating urination or urinary retention
- Numbness in the saddle area (groin, buttocks, inner thighs)
- Rapidly worsening leg weakness
- Severe back pain with significant neurological deterioration
These symptoms require same-day emergency assessment and urgent MRI.

- Increasing difficulty walking or balance problems

These presentations should be escalated following local spinal pathways (e.g., National Back Pain and Radicular Pain Pathway / MSK triage systems) for further assessment and possible specialist referral.

However, the purpose of recognising these symptoms is not to cause alarm — it is to ensure that the small proportion of people who do need urgent investigation are identified quickly and appropriately referred through the correct clinical pathway.
